Position Overview:
The Transition Coordinator serves as a key liaison among Orange-Ulster BOCES, component school districts, local businesses and service agencies to support the coordination of transition services for students with disabilities. This position is focused on improving outcomes in employment, independent living, education, and community involvement by addressing service gaps, building interagency collaboration, and promoting effective transition practices throughout the region.
Job Responsibilities:
Identify gaps in current transition services and work collaboratively with stakeholders to develop new programs, resources, and initiatives that meet student needs.
Develop, research, and disseminate tools and materials based on best practices in transition planning for students with disabilities.
Prepare and submit grant applications to secure funding for regional transition services and initiatives.
Attend and participate in local, regional, and state transition planning meetings and events.
Maintain documentation and prepare written reports, correspondence, and summaries of work activity as required.
Utilize technology including spreadsheets, word processing, email, calendar systems, and databases to manage workflow and communications effectively.
Perform additional related duties that support successful student transitions, as assigned.
Required Knowledge, Skills, and Abilities:
Solid understanding of the needs, interests, and characteristics of students with disabilities.
In-depth knowledge of available transition services provided by public and private service agencies.
Strong ability to analyze service delivery needs and coordinate programming across multiple entities.
Skill in program development, training delivery, and public relations.
Proficiency in researching and developing grant proposals.
Effective oral and written communication skills.
Ability to build and maintain productive relationships with schools, families, agencies, and community organizations.
Competency in using software applications, including word processing, spreadsheets, calendars, email, and databases.
Demonstrated initiative, sound judgment, creativity, and resourcefulness.
Physical condition commensurate with the responsibilities of the position.
